Traditional management, such as intermittent grinding, is no longer recommended due to its unreliability and the risk of inducing inflammation.
Modern management shifts toward prophylactic Vital Pulp Therapy (VPT) to ensure continued root development. The “Prep & Fill” technique demonstrates a superior 95.79% success rate compared to resin reinforcement alone. Given that approximately 10% of DE tubercles involve pulpal extensions, immediate VPT using biocompatible materials is recommended to maintain pulp vitality and stimulate tertiary dentin formation. While CBCT enhances the diagnosis of complex pulpal extensions, clinicians must balance its benefits against pediatric radiation sensitivity. This presentation advocates for a biology-driven approach that moves beyond the tubercle to preserve the structural integrity of affected teeth.
